Our care approach is patient-centered and involves an integrative program that addresses the complete healthcare needs of our patients. We assemble an interdisciplinary team of practitioners from several areas to design customized treatment plans that reflect the physical and psychosocial needs of each individual.
A committed, dynamic team of case managers, physicians, dieticians and nutritionists, social workers, recreation therapists, registered nurses, licensed practical nurses and certified nursing assistants work together to optimize the healthcare outcomes of each patient. Carillon Nursing and Rehabilitation Center is a 315 bed skilled nursing facility.
