If you are interested in coming to Carillon Nursing and Rehabilitation Center, our admissions team can help you get started. They will provide guidance to you as you seek placement in our sub-acute care and rehabilitation and skilled nursing programs, and they will also work closely with your social worker as you transition back to your home and community.

Our admissions team will help you understand our policies and procedures, address any unresolved questions, and ensure that you feel very comfortable with every aspect of the process. Should you wish, a tour of the facility can be provided at your convenience.

Eligibility

We welcome all candidates who are at least 18 years of age who qualify for nursing home placement, sub-acute care, or rehabilitation. Our admissions team will work with you to confirm your ability to heal with us, and will require a completed application, a Patient Review Instrument, and Screen.

If you are interested in Medicaid coverage, we have a Medicaid Coordinator who can help you determine your Medicaid eligibility, and work with you on the application process.
